CHW Sacramento Service Area hospitals and other local hospitals work together to identify and collaborate on initiatives that address needs identified through the Community Needs Assessment by increasing access to primary and specialty care, and social services to underserved populations.
Through the support of these partners, a number of programs have been founded to meet critical needs, including:
