CHW Community Grants Program

The right resources in the right place at the right time  |  We can't change the world by ourselves. Partnering with others who share our vision and values is the only way to bring about real improvements in the healthcare system. Our Community Grants program is one way we are working with others to increase access to quality care.
 
Organizations that receive grant funds from us are working to improve access to jobs, housing, food, education and healthcare for people in low-income and minority communities.
 
Since this program's inception in 1990, over $38 million has been awarded to help support over 2,300 health and health-related programs.
 
 
Our Priorities  |  We are looking to partner with other nonprofit organizations whose programs respond to the strategic priorities identified in our hospitals' Community Health Assessments or Community Benefit Plans. Grant funds are to be used to provide services to underserved populations.
 
The depth of the recession has devastated the public healthcare safety-net. Budget reductions have stretched Sacramento and surrounding counties' primary, mental and public health services to the limit and placed thousands of low-income and vulnerable residents at risk.
 
The 2011 CHW Community Grants Program for the Sacramento Service Area is structured to strengthen our region's healthcare safety-net. Grant proposals for Sacramento, El Dorado and Placer Counties must  address the following priorities:
  1. Established partnership programs with community-based primary care clinics to offset the cost of imaging, laboratory and other testing services, and to support sliding-scale fees
  2. New initiatives that build capacity to increase access to primary, mental and specialty (i.e. dental, women and children) care for uninsured, underinsured and undocumented immigrants
  3. Programs that provide wraparound care and services, including transitional and permanent housing for the homeless; substance abuse treatment; case management and referral support; transportation; and other complementary programs to underserved populations
 
2011 Grant proposals for Yolo County must  address the following priorities:
  1. Initiative that provide access to primary, mental and specialty (i.e. dental, women, children and elderly) care for uninsured, underinsured and undocumented immigrants and vulnerable populations
  2. Providing wraparound care and services, including transitional and permanent housing for the homeless; substance abuse treatment; mental health services; case management and referral support; transportation; and other complementary programs to underserved populations
 
2011 Grant proposals for Nevada County must address the following priorities:
  1. Programs that provide services for senior health and youth at risk
 
Criteria
  • Grant funds were awarded up to $50,000
  • Applicants must strictly adhere to the grant guidelines
  • Proposals must identify the type of change expected and how, and over what time period, progress will be measured
Eligibility Requirements
  • Applicant must be a 501(c)(3) nonprofit organization or have a fiscal agent that is a 501(c)(3) organization and capable of administering the project's funds
  • The project/program must be a response to the health priorities identified in the local CHW hospital's community assessment or Community Benefit Plan
  • The project must involve collaboration with other organizations and/or with a CHW hospital

Workshop mandatory for applicants from Sacramento, El Dorado and Placer Counties. Applicants from Yolo and Nevada Counties strongly encouraged.

Invitations to submit full proposal mailed.

Notification of Grant recipients.
