Call for Letters of Intent
August 3, 2009 - Catholic Healthcare West (CHW) Sacramento Service Area Hospitals will award over half a million dollars to nonprofit organizations during the 2009 CHW Community Grants Program funding cycle now under way. Letters of Intent are being accepted through August 21, 2009.
The annual grants program is one of the ways in which CHW works together with its communities to bring about improvements in health and quality of life. Grants are funded by contributions from CHW member hospitals, and must address the priority needs of underserved populations that are identified through a community needs assessment. Since the program's inception in 1990, nearly $28 million has been awarded to help support 1,800 health and health-related programs.
FUNDING CRITERIA
Specific grant priorities have been established for 2009 grants in direct response to the economy and major budget cuts by county and state governments that have resulted in a significant reduction in the region's health care assets and health services for those in need. Funding will be awarded in two categories:

Programs that provide access to primary, specialty, and mental and dental health care for uninsured, underinsured and undocumented immigrants.

Programs that provide wraparound care and service, including transitional and permanent housing for the homeless; substance abuse treatment; case management and referral support; transportation; and other complementary programs to underserved populations.
Funding up to $25,000 will be considered for eligible organizations that meet one of these priorities. Collaborative efforts with CHW hospitals and/or other community-based organizations are strongly encouraged.
In addition, grants up to $5,000 will be considered for general program proposals that integrate one or more of the five core principles that guide CHW community benefit practices:
- Programs that focus on disproportionate unmet health-related needs;
- Programs that emphasize primary prevention, address underlying causes of health problems, and help with management of chronic illness;
- Programs that contribute to a seamless continuum of care;
- Programs that build community capacity; and
- Programs that emphasize collaboration.
ELIGIBILITY
- Organizations must provide services to residents within the counties of Sacramento, Yolo, El Dorado, Placer and Nevada that are served by CHW hospitals including Mercy General, Mercy San Juan Medical Center, Methodist, Mercy Hospital of Folsom, Woodland Healthcare and Sierra Nevada Memorial.
- Applicants must be a 501(c)(3) nonprofit organization, or have a 501(c)(3) fiscal agent capable of administering program funding.
- Applicants may not apply if they have received grants in 2009 from Mercy Foundation or the Sisters of Mercy.
- Funding cannot be used for planning grants or research or to create a new coalition.
- Organizations must be able to define how the benefits/outcomes of their program will be measured in order to demonstrate results.

The original Letter of Intent must be mailed or hand delivered to the attention of Rosemary Younts, Community Benefit Director, at Catholic Healthcare West, 3400 Data Drive, Rancho Cordova CA 95670. A copy of the Letter of Intent must also be emailed to Younts at her above email address. Deadline is 5 p.m. on August 21, 2009. Any Letter of Intent received after the deadline will not be accepted. Organizations will receive an email confirming receipt of their Letter of Intent.
A local Grants Review Committee will review all Letters of Intent by September 14, and will then request full proposals from those organizations whose programs best meet the priorities described.
